What is the Logo or design required for decoration:

Screen Print, Embroidery or Digital Transfer, Other
Item or Product Number if known
For example: T Shirt GD5000, Polo Shirt SS11etc
Specific Brand if required
Gildan, Fruit of the Loom, Henbury, Premier etc

Colour:

Colour of garment. ( light or dark )

Item Quantities:

100 T shirts, 50 Polos

Position Print/Embroidery:

Front Chest, Left Breast, Full back, Right/Left Sleeve etc

Print Colours: Max 10

Number of colours with Pantone references if possible

Embroidery Colours: Max 15

Pantone or shade card reference if known Isocord or Madira threads, other.

Artwork Format:

Adobe Illustrator format (CS2 or less) or vector graphic all text to curves or paths, High Resolution J Peg.

Repeat Orders:

Please give last order acknowledgement number
